* Add blend microcode registers

* Add advanced blend support using host extension

* Remove debug message

* Use pre-generated table for blend functions

* XML docs

* Rename AdvancedBlendMode to AdvancedBlendOp for consistency

* Remove redundant code

* Fix some advanced blend related issues on Vulkan

* Formatting
